By accepting this registration, I hereby release EPIC Twirl West Coast, (“EPIC”), Leah Stoffel, and/or their officers, employees, faculty, instructors, servants, agents and assigns from all liability for personal injury or illness while enrolled at EPIC. I understand that baton twirling, dance, and exercise may cause injury and I certify that my child is in good health and is capable of participating in classes. EPIC has my permission to use photos of my minor child taken at classes and events in print form and/or electronically, including on the EPIC Twirl Facebook, Instagram, and website. If I do not want photos of my minor child shared in print form and/or electronically, I understand that it is my responsibility to advise in writing prior to photos being posted.
